United Sevice Association offers dental plans through the Ameritas Dental network in the Valu+ benefits package. This dental plan is $59.00 per month for an individual and $69.00 per month for a family.

Ameritas Group Dental
This group insurance plan features 241 covered procedures, and pays up to $1,000 annually less a $50 deductible for preventative services and a $100 deductible per family member for basic and major services, (maximum of 3 deductibles annually). There is no waiting period for preventive and basic services; major services require a 12-month waiting period.

Full freedom of choice allows USA+ Members to use any dentist they choose and be reimbursed according to predetermined benefit amounts for each procedure, or visit one of 55,000 Ameritas participating providers where claims can be submitted by the dentist directly.
